Welcome to Smile Craft. These Terms of Service ("Terms") govern your use of our website and dental services. Dental Services

Smile Craft provides general and cosmetic dental services. Our services include but are not limited to:

Appointments

Scheduling: Appointments can be scheduled online, by phone, or in person. We recommend scheduling in advance to ensure availability.

Cancellation Policy: We kindly request at least 24 hours notice for appointment cancellations or rescheduling. Repeated no-shows or late cancellations may result in a cancellation fee or dismissal from the practice.

Late Arrivals: If you arrive late for your appointment, we will do our best to accommodate you. However, your appointment may need to be shortened or rescheduled to avoid delays for other patients.

Treatment Consent

Before any dental treatment, you will be informed of the proposed procedures, alternatives, risks, and expected outcomes. Your written consent will be obtained for significant procedures. You have the right to ask questions and receive answers before consenting to treatment.

By consenting to treatment, you acknowledge that:

Payment and Insurance

Payment: Payment is due at the time of service unless other arrangements have been made. We accept cash, checks, and major credit cards.

Insurance: We accept most major private insurance plans. As a courtesy, we will submit claims to your insurance company on your behalf. However, you are responsible for understanding your insurance benefits and for any balance not covered by insurance.

Estimates: Treatment cost estimates are provided as a courtesy and are not a guarantee of final costs. Actual costs may vary based on findings during treatment.

Outstanding Balances: Accounts with balances over 90 days may be subject to collection action. Any collection costs or legal fees incurred will be the patient's responsibility.

Patient Responsibilities

As a patient of Smile Craft, you agree to:

• Follow post-treatment instructions and care recommendations
